Bile Culture in Biliary Tract Disease

Abstract
Following surgery of biliary tract disease, biliary bacteria was associated with development of inflammatory complications caused by antibiotics-resistant bacteria due to the indifferent abuse of antibiotics.
So we analyzed 202 cases of biliary tract disease which had been performed at National Medical Center during the period from January 19/9 to December 1990.
@ES The results were as follows;
@EN 1) Overall positive culture rate was 80.7%(163 cases).
2) The most common isolated organinsm was E. coli and Gram(-) organism was mainly isolated (93.1%).
3) Anaerobic bacteria was found in one cases(Bacteroides fragilis).
4) Significant clinical factors contributing to positive culture were leukocytosis, biliary stone and older age(over 60 y-o).
5) Antibiotics effective to Gram(-) organisms were Tobramycin, Cefotaxim, Gentamicin Amikacin and kanamycin, and antibiotics effective to Gram(+) organisms were Ampicillin, Amikacin, Chloramphenicol and Tetracycline.
6) There was positive correlation between positive bile culture and postoperative infectious complications.
We concluded that prophylactic broad-spectrum antibiotics may reduce postoperative infections complications and intraoperative bile culture and sensitivity test have the significant in formation of postoperative antibiotic therapy.
